Consent to processing of personal data
Expressing my consent freely, of my own free will and in my own interest, in accordance with the Federal Law of 27.07.2006 № 152-FL "On Personal Data" (hereinafter — the Law № 152-FL), by ticking the box (check-box) "I consent to the processing of personal data" when submitting an application on the website https://aikkmp.com/ I hereby give my consent to the Association of Importers of Cosmetology, Cosmetic and Medical Products "AICCMP", legal address: 119180, Moscow, vn.ter.g. municipality district Yakimanka, Bolshaya Polyanka St., 51A/9, room. 1/1/8 (hereinafter — the Organization, the Operator), to process my personal data for the purpose of organizing admission to membership of the Association "AICCMP" or inclusion in the list of partners, notifying me of the results of consideration of appeals, applications and information messages submitted by me, providing me with access to services available to members or partners of the Association "AICCMP", preparation, conclusion and execution of civil law contracts with partners.
List of actions with personal data for which consent is given: processing of the above personal data will be carried out through mixed (automated, non-automated) processing of personal data, including transmission via communication channels, any action (operation) or set of actions (operations) performed with or without the use of automation tools, including collection, recording, systematization, accumulation, storage, clarification (update, change), extraction, use, transfer (distribution, provision, access), depersonalization, blocking, deletion, destruction.
The Operator has the right to process my personal data by entering them into an electronic database, including them into lists (registers) and reporting forms stipulated by the legislation and/or documents regulating the procedure for keeping and composition of data in accounting and reporting documentation, including contracts concluded with third parties. I hereby express my consent to the Operator to entrust the processing of my personal data to third parties in accordance with the purpose of this consent, subject to their compliance with the principles and rules of personal data processing stipulated by applicable law. Third parties undertake to ensure the security of personal data during their processing and prevent disclosure of personal data, as well as have the right to perform actions similar to those of the Operator.
List of persons to whom personal data may be transferred or assigned for processing:
1) the Federal Service for Supervision in the Sphere of Healthcare (Roszdravnadzor), address: 109012, Moscow, Slavyanskaya Square, pl. 4, p. 1, Federal State Budgetary Institution "All-Russian Research and Testing Institute of Medical Technology" of Roszdravnadzor, address: 115478, Moscow, Kashirskoye Shosse, 24, p. 16, as well as licensing and/or controlling public authorities and local authorities, municipal authorities; state financial control authorities, including auditors, and other federal authorities in accordance with the legislation of the Russian Federation, in order for the Organization to form reports on the activities of the Association "AICCMP";
2) in order to ensure the processing of appeals and applications submitted by me — to physical and legal entities under civil law contracts.
I confirm that my rights and obligations in the field of personal data protection have been explained. I confirm that I am familiarized with the provisions of the Law No. 152-FL, as well as with the Operator's Personal Data Processing Policy.
I am aware that full or partial withdrawal of this Consent, as well as termination of personal data processing provided for by this Consent may result in the impossibility to achieve the purpose of personal data processing provided for by this Consent.
In case of withdrawal of consent to personal data processing, the Operator has the right to continue personal data processing if there are grounds specified in paras. 2-11 of part 1 of Article 6, part 2 of Article 10 and part 2 of Article 11 of Law no. 152-FL.
This Consent to the processing of personal data is valid from the day of its signing until the purpose of personal data processing is achieved, or until the day of revocation in writing sent from the e-mail address specified during registration to the official e-mail address of the Operator info@aikkmp.com, unless otherwise provided by the Law No. 152-FL.
I confirm that I have given my consent to send advertising, informational mailings from the Organization and its authorized persons to the e-mail address specified by me in order to inform about the results of consideration of my appeals and applications, about events held by the Organization and other issues related to membership/partnership in the Association "AICCMP".
Information about the person who signed (provided) this Consent shall be indicated when submitting a request or application in the field (checkbox) "I consent to the processing of personal data" on the website https://aikkmp.com/. The Consent shall be signed by ticking the appropriate field (checkbox).
